Welcome to Kansas Pediatric Neurology Center!

Before You Visit

For new patients, you may print and complete our new Patient Registration Forms and bring them to your appointment. This will expedite registration at the time of your visit. Established patients are required to renew these forms every two years. Additional forms may be required and will be provided at your appointment.

  • If your plan needs a referral please make sure to obtain one from your primary care provider before your appointment.
  • Please bring copies of any relevant medical records, laboratory reports, EEG reports, MRI and CT scans for the doctor to review.
At The Time Of Your Visit

Things to bring with you to each visit:

  • Current insurance card(s)
  • Photo identification
  • Bring relevant records, scans, and your referral (if applicable)
  • Your preferred method of payment for any cost shares due at the time of service

Please ask our staff to help you enroll in our web-based Patient Portal. Patients can use the Patient Portal to view, download, and transmit their health information, request prescription refills, send secure messages to their providers, and pay their bills.

If you need to cancel your visit, we require one business day’s notice.
